Our team is led by NP Eliza Rivera-Mitu, who holds a dual Board Certification in Advanced Diabetes Management and as a Family Nurse Practitioner. Ms. Rivera-Mitu is a 11-year Nurse Practitioner with experience in Primary Care, emphasizing in prevention and management of Diabetes, Hypertension, Hyperlipidemia, Obesity, and other chronic diseases. She is also experienced in Addiction Medicine, Cardiology, Pediatrics, and Hospice Care. As a veteran RN with 31 years of experience, she is also experienced in home health care, gastroenterology, long-term pediatric care, and Nursing education. She is also a Certified Health Coach, who emphasizes in weight management and weight loss.
Eliza is fluent in Tagalog and knows conversational medical Spanish.
Our mission is to stand in the gap for those who do not have immediate access to health prevention and management through telemedicine and home visits. We also aim to be an agent of change in the areas of Obesity, Diabetes care, and Chronic Disease management. Our vision is to have a significant impact in decreasing the mortality and improving the overall health of our patients.
Brian Mitu, Nurse Practitioner and Board-Certified Physician Assistant, received his Associate of Science degree in nursing from Long Beach City College in 1998. He completed a dual certification program as a primary care nurse practitioner and physician assistant from Stanford University in 2006.

His practice philosophy centers on preventative medicine and personalized care, emphasizing the importance of proactive health management tailored to each individual. Throughout his career, Brian has demonstrated a commitment to staying abreast of the latest medical advancements and techniques, ensuring his patients receive the highest quality of care.
He approaches each patient interaction with empathy, compassion, and a genuine desire to improve their overall well-being. When Brian is not taking care of patients he likes to stay fit through metabolic conditioning and weight training. He is fluent in Tagalog.
