• By opting into SMS from a web form or other medium, you are agreeing to receive SMS messages from Angeles Health and Wellness Center. This includes SMS messages for customer care, marketing. 1. SMS Consent Communication

Phone numbers collected during the SMS opt-in process are not shared with third parties for marketing purposes.

If you’ve consented to receive SMS messages from Angeles Health and Wellness Center, you may receive texts related to:

You may receive up to 4 SMS messages per week, depending on appointment schedules, billing reminders, and service-related updates.

 Standard message and data rates may apply, depending on the carrier's pricing plan. These fees may vary if the message is sent domestically or internationally. 

5. Opt-In Method

You may opt in to receive SMS messages by:

• Checking the SMS consent box on our intake forms.
• Providing your mobile number and consent verbally or in writing during your visit or over the phone.

Sample Opt-in message: Thank you for opting into SMS messaging from Angeles Health and Wellness Center. Message frequency varies. To opt out, text STOP. For assistance, text HELP or visit https://angeleshealthandwellness.com/. Message & data rates may apply.   

6. Opt-Out Method

You can opt out at any time by:

• Replying “STOP” to any SMS you receive.
• Contacting us directly to request removal from our SMS list.

Sample Opt-out message:  You will no longer receive messages from Angeles Health and Wellness Center. To opt back in at any time reply START.
